Crouse Health Hospital – Commonwealth Place, a distinguished rehabilitation center situated in Syracuse, New York, is committed to the treatment of individuals aged 18 and older who are affected by substance use disorders, co-occurring disorders, and mental health issues. The Opioid Treatment Program (OTP) at Crouse Health provides medication-assisted treatment for individuals who are afflicted with opioid use disorders. This treatment involves the use of medications such as methadone, buprenorphine, and naltrexone. This program is specifically intended to provide assistance to individuals who are dependent on opioids such as fentanyl, hydrocodone, oxycontin, and heroin. The program offers an integrated recovery plan that encompasses medical, psychiatric, and psychosocial support to guarantee that each client receives the necessary care. In order to qualify, clients must satisfy specific state and federal requirements, which include completing a comprehensive assessment and physical examination.

Crouse Health offers Withdrawal Management Services at its hospital facility for patients who require medical assistance with alcohol withdrawal. The emergency department conducts a thorough evaluation of patients who are enrolled in this service to guarantee a personalized care plan and comprehensive assessment. Patients are referred for further treatment with the assistance of an interdisciplinary team after their time in withdrawal management, thereby facilitating a smooth transition to ongoing rehabilitation. Crouse’s Addiction Consult Services (ACS) are designed to identify patients who are hospitalized for non-addiction-related medical conditions and may benefit from additional addiction support. This service facilitates the provision of appropriate referrals and treatment recommendations to address substance use needs holistically. 

Commonwealth Place Inpatient Rehabilitation Program provides a 40-bed residential facility that provides supervised care 24 hours a day in a structured environment. This program prioritizes personalized treatment, which includes gender-responsive programming, individual and group therapy, psychiatric consultations, and educational seminars that cover life skills, relapse prevention, and substance use. The program also places a substantial emphasis on family involvement, acknowledging the critical role of family support in the successful recovery process. Clients collaborate with their counselor to establish a comprehensive aftercare strategy prior to discharge, which guarantees continuity of care as they transition to community resources or outpatient programs. 

Crouse Health also provides Vocational Rehabilitation Services and Peer Recovery Services to facilitate clients’ long-term recovery. Certified Recovery Peer Advocates offer mentorship and guidance, assisting clients in navigating challenges that affect their recovery and connecting with community resources. Vocational Rehabilitation Services are administered by Certified Rehabilitation Counselors, who collaborate with patients to establish and accomplish independent living and career objectives. These services encompass referrals to community agencies for ongoing support in living independently, as well as assistance with employment applications, resume writing, and interview preparation.
